Hospice At Home Programme

Pain Management

Our team specialises in assessing and managing pain to ensure patients are comfortable and can maintain quality of life.

Medication Review and Management

We regularly review medications to ensure they are effective and appropriate for symptom control. Personalised care plans include adjustments based on the patient’s needs and condition.

Symptom Control

Beyond pain, we address other distressing symptoms such as breathlessness, nausea, and fatigue. Our holistic approach ensures physical and emotional comfort for patients.

Dedicated Support

Each patient is assigned a dedicated nurse who will be deeply familiar with their case. A consistent point of contact can help provide reassurance and stability during a time of uncertainty.

Caregiver Guidance and Training

Caregivers are a vital part of the patient’s support system. We provide practical training and ongoing guidance to help them confidently manage daily care tasks.

Coordination of Care

Our team works collaboratively with the patient’s primary care physicians, specialists, and other healthcare providers to ensure a seamless continuum of care.

Palliative care is an approach that improves the quality of life of patients who are facing problems associated with advanced life-limiting illnesses. Palliative care uses a multi-disciplinary team approach to support patients and their caregivers. It prevents and relieves suffering through the early identification, correct assessment and treatment of pain and other physical, psychosocial or spiritual concerns.

Palliative care is required for a wide range of diseases. The majority of adults in need of palliative care have chronic diseases such as:

Our Hospice at Home Programme is free of charge for the public. Additionally, we provide medical equipment loans for free*. 

*Equipment is subject to availability and eligibility criteria. We do charge a refundable deposit for equipment loans. Please contact us for more information.
